sech1He sent 350 XMR (7x50 XMR withdrawals) to the quick swap service directly from the exchange, 3 of those withdrawals were combined in a single tx (the swap service didn't expect this edge case), things got messy of course... Rule #1 is to never keep money on exchanges, rule #2 is to never send money directly between exchanges
